metadata.dc.description.resumo: | EVOLUÇÃO DA DISFUNÇÃO ERÉTIL EM INDIVÍDUOS INFECTADOS PELO HTLV-1. A disfunção erétil (DE) em indivíduos infectados pelo vírus linfotrópico de células T humano do tipo 1 (HTLV-1) tem forte associação com as disfunções neurológicas causadas por essa infecção. Estudos avaliando outros fatores de risco para a DE como arteriosclerose, fatores hormonais e fatores psicogênicos evidenciaram que esses fatores têm pouca influência na DE em indivíduos com HTLV-1. O objetivo desse estudo foi avaliar a evolução da DE em indivíduos infectados por esse vírus. Métodos: estudo de coorte prospectivo de janeiro de 2004 até junho de 2019 com indivíduos do sexo masculino, com DE, infectados pelo HTLV-1, com idade entre 18 e 70 anos, sem história de câncer, sem uso de prótese peniana, sem anemia falciforme e sem déficit motor causado por outra patologia. Foi avaliado o grau de DE com o Índice Internacional de Função Erétil 5 (IIEF-5), o grau de lesão neurológica foi avaliada com a Escala Expandida do Estado de Incapacidade (EDSS) e a Escala Motora de Incapacidade de Osame (OMDS), o grau de hiperatividade detrusora pela Escala de Sintomas de Bexiga Hiperativa (OABSS), e a carga proviral do HTLV-1 durante o acompanhamento no ambulatório. Resultados: Participaram da coorte 90 indivíduos com idade média de 52,8 ± 9,78 anos. No início do estudo, 42 desses eram portadores de HTLV-1, 16 tinham provável HAM/TSP e 32 tinham HAM/TSP definida. A mediana de acompanhamento foi de 8,5 (3-12) anos. Foi observado que o valor do IIEF-5 era maior nos indivíduos portadores do que nos indivíduos com sintomas neurológicos e houve uma correlação inversa entre o IIEF-5 e o OABSS no seguimento do estudo (r-0,62, P<0,001). Durante a coorte, o IIEF-5 diminuiu entre os portadores e mais ainda nos prováveis HAM e nos HAM/TSP definido (P<0,001) evidenciando a piora da DE em todos os 14 grupos. Em uma análise de sobrevivência, o tempo para desenvolver DE grave foi menor nos indivíduos com HAM/TSP comparado aos indivíduos portadores do vírus (P=0,001) e também comparado com os prováveis HAM/TSP (P=0,014). A presença de HAM/TSP definida no início do estudo foi independentemente associada com o desenvolvimento de DE grave, mesmo após ajustes para idade basal e carga proviral (HR 6,74; P = 0,008). Conclusão: A DE é uma manifestação clínica lenta e progressiva da infecção pelo HTLV-1 e está fortemente associada com o grau de comprometimento neurológico da doença. O desenvolvimento da DE pode servir de sinal de alerta para uma possível degeneração neurológica. |
Abstract: | Background: Virtually all patients with HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) have some degree of erectile dysfunction (ED) but ED is also found in a large percentage of HTLV-1 carriers. Aim: To evaluate the evolution of ED in HTLV-1 infected individuals followed for up to 15 years. Methods: Prospective cohort study of HTLV-1 infected men with ED, aged 18 to 70 years, followed from January 2004 to December 2019. We used the International Abbreviated Index of Erectile Function 5 (IIEF-5), the Expanded Disability Status Scale (EDSS) and the Osame Motor Disability Scale, and the Overactive Bladder Symptom Score (OABSS) to define and stratify ED, neurologic and bladder dysfunction, respectively. Outcomes: Time to development of severe ED. Results: We studied 90 men with ED, mean age 52.8 ± 9.78 years. At baseline, 42 were carriers, 16 had probable HAM/TSP and 32 had definite HAM/TSP. IIEF-5 was highest among carriers and lowest in patients with definite HAM/TSP whereas OABSS was lowest in carriers and highest in definite HAM/TSP patients. Median follow-up was 8.50 [3.00 – 12.00] years. IIEF-5 fell significantly from baseline to last follow-up among carriers, probable HAM/TSP and definite HAM/TSP patients. There was an inverse correlation between the IIEF-5 and the OABSS at last follow-up (r = -0.62, P < 0.001). In survival analysis, the time to development of severe ED was significantly shorter in patients with definite HAM/TSP when compared to carriers (P = 0.001) and probable HAM/TSP (P = 0.014). The presence of definite HAM/TSP at baseline was independently associated with the development of severe ED, after adjustment for baseline age and proviral load (HR 6.74; P = 0.008). Clinical implication: Formal assessment of erectile 52 function should be part of the routine clinical assessment of HTLV-1-infected individuals; worsening erectile function should alert clinicians to the possibility of neurologic deterioration. Strengths and limitations: This is the first prospective cohort study to describe the course of ED in HTLV-1-infected individuals. The small sample size and absence of seronegative controls are limitations. Conclusion: ED is a slowly progressive clinical manifestation of HTLV-1 infection and the degree of neurologic compromise at baseline is the main predictor of time to progression to severe ED. |
