Impacto aos seis meses de vida de orientações individuais sistematizadas no aleitamento materno de recém-nascidos que necessitaram de internamento hospitalar ao nascimento: ensaio clínico randomizado

Abstract

OBJETIVO: Avaliar o impacto, aos seis meses de vida do recém-nascido, de orientações individuais em puérperas com recém-nascido que necessitaram de internamento em uma unidade de cuidados intermediários, logo após o nascimento. MÉTODO: Trata-se da segunda fase de um ensaio clínico, inicialmente desenvolvido para avaliar o impacto de orientações individuais a curto prazo, através da atitude materna em amamentar durante hospitalização. Após alta os participantes foram acompanhados em uma coorte, através de contato telefônico mensal até seis meses de vida. As variáveis que fizeram parte do acompanhamento foram aleitamento materno exclusivo, qualquer oferta de leite e possíveis variáveis que pudessem interferir nos resultados ao longo da intervenção. RESULTADOS: 159 puérperas e recém nascidos iniciaram o acompanhamento, divididos em três grupos (controle, orientações individualizadas e orientações pode vídeo) com características semelhantes. Na alta o número de puérperas que estavam amamentando exclusivamente foi maior no grupo que recebeu orientações individualizadas, 91,07% das mulheres. A taxa de interrupção do aleitamento materno exclusivo foi menor no grupo intervenção com orientação individual, sendo mais frequente após a alta, no primeiro mês de vida. A variável chupeta e mamadeira, quando introduzida apresentou relação significativamente estatística com a interrupção do aleitamento materno exclusivo. CONCLUSÃO: Orientações individualizadas apresenta desfecho favorável para o aleitamento materno exclusivo, aumentando a frequência de aleitamento materno exclusivo e qualquer oferta de leite materno na alta e durante acompanhamento até o sexto mês de vida.

OBJECTIVE: To assess the impact, at six months of the newborn's life, of individual guidance on mothers with newborns who needed hospitalization in an intermediate care unit, soon after birth. METHOD: This is the second phase of a clinical trial, initially developed to assess the impact of short-term individual guidance, through the maternal attitude of breastfeeding during hospitalization. After discharge, participants were followed up in a cohort, through monthly telephone contact for up to six months of life. The variables that were part of the follow-up were exclusive breastfeeding, any milk supply and possible variables that could interfere with the results throughout the intervention. RESULTS: 159 postpartum women and newborns started follow-up, divided into three groups (control, individualized guidance and video guidance) with similar characteristics. At discharge, the number of mothers who were exclusively breastfeeding was higher in the group that received individualized guidance, 91.07% of women. The rate of interruption of exclusive breastfeeding was lower in the intervention group with individual guidance, being more frequent after discharge, in the first month of life. The variable pacifier and bottle, when introduced, showed a statistically significant relationship with the interruption of exclusive breastfeeding. CONCLUSION: Individualized guidelines have a favorable outcome for exclusive breastfeeding, increasing the frequency of exclusive breastfeeding and any supply of breast milk at discharge and during follow-up until the sixth month of life.
