O sentido do ser-mulher-puérpera no método mãe canguru

Abstract

O Programa de Atenção Humanizada ao Recém-nascido Prematuro e de Baixo Peso, mais conhecido como Metodologia ou Método Mãe Canguru (MMC), incorporado por várias maternidades brasileiras, preconiza precoce e constante contato pele a pele entre mãe e recém-nascido, pelo tempo que ambos demonstrem ser prazeroso e suficiente. Tem como objetivos: aumentar o vínculo mãe-filho; estimular o aleitamento materno; melhorar o controle térmico de prematuros; reduzir o número de recém-nascidos em unidades de cuidados intermediários; diminuir os índices de morbi-mortalidade neonatal e favorecer aos pais maior competência e confiança, quando do cuidado domiciliar. É uma estratégia do Ministério da Saúde que prioriza a atenção ao recém-nascido, mas que tem como protagonista a mãe. Vivenciando o cotidiano hospitalar com as mães no MMC, percebia-se a ansiedade quanto ao afastamento do convívio familiar, medo do agravamento do quadro clínico do recémnascido, a culpa pelo nascimento prematuro, os momentos de explosão de alegria e/ ou tristeza pelo ganho ou perda ponderal do (a) filho (a), a preocupação com a vida conjugal, situações somadas à multiplicidade de papéis, agora socialmente mais cobrados, devido a nova constituição familiar. Este estudo fenomenológico tem como objeto o sentido do ser-mulher-puérpera no Método Mãe Canguru e objetiva compreender a vivência de mães na 2ª fase do MMC. O referencial teórico é baseado em estudiosas (os) da temática e o referencial teórico-filosófico foi iluminado pelo pensamento heideggeriano expresso em Ser e Tempo. A entrevista fenomenológica foi realizada no alojamento canguru de um hospital público do interior do Estado da Bahia, tendo como depoentes mães inseridas na segunda etapa do MMC. As questões que nortearam a entrevista foram: Como é para você ser mãe deste bebê prematuro? Como tem sido participar do Método Mãe Canguru? Na interpretação hermenêutica, compreendeu-se que o sentido de ser-mulher-puérpera no MMC, em seu cotidiano vivencial, foi desvelado pelo temor, pelo falatório, pela solicitude, pela angústia, modos de ser essencialmente ligados à temporalidade expressos nas falas, nas lágrimas, nos sorrisos, nos silêncios, enfim, nas situações que mostraram o modo de ser de cada uma. O estudo poderá beneficiar a unidade hospitalar envolvida, a estruturar melhor as etapas desta metodologia de humanização, conhecendo o significado em ser-mulher-puérpera no MMC, além de contribuir para ampliar a reflexão da comunidade acadêmica e de saúde quanto à forma de implementação do Método. Destaco a importância deste Método para a saúde da criança e para o aumento do vínculo familiar. Todavia, há de se considerar que, para seu pleno êxito, a vivência da mulher nele inserida deva ser valorizada.

The Humanized Attention Program to the Premature and Low Birth Weight Infant, more well known as Kangaroo Mother Care Method or Methodology ( KMC), was incorporated by many Brazilian maternity hospitals. It advocates early and constant skin-to-skin contact between mother and newborn, by the period that both of them demonstrate to be pleasurable and sufficient. Its objectives are: to increase the mother and child link; to stimulate the maternal breast-feeding; to improve the thermal control of premature children; to reduce the number of newborns in intermediate care units; to diminish the rates of neonatal morbi-mortality; and to give to the parents more competence and confidence, when dealing with the home care. It is a strategy of the Health Department that advocates the new-born, but has as its protagonist the mother. Experiencing the hospital everyday life with the mothers in KMC, I realized the anxiety concerning the distance in the familiar involvement, the fear of the aggravation of the newborn clinical panorama, the guilt by the premature birth, the moments of explosion of happiness and or sadness for the gain or loss of the child, the worry with the conjugal life, situations added to the multiplicity of roles, now socially more required, due to the new familiar constitution. This phenomenological study that has as its object the sense of being a puerperal woman in the Kangaroo Mother Care method, and its objective is to understand the mothers experience in KMC. The bases of the theoretical referential are studious of the theme and the philosophic-theoretical referential was enlightened by the heideggerian thought expressed in Being and Time. The phenomenological interview took place in the kangaroo accommodation in a public hospital in a Bahia State interior, having the declarations of the mothers inserted in the second phase of KMC. The following questions that directed the interview were: How do you feel being the mother of a premature baby? How do you experience your participation in the Kangaroo Mother Care method? In the hermeneutic interpretation, comprehended that the sense of being a puerperal woman in KMC in her everyday life was uncovered by the fear, by the talk, by the helpfulness, by the anguish, ways of being essentially linked to the temporality, expressed in the speeches, in the tears, in the smiles, in the silence, anyway, in situations that showed the way of being of each woman. The study can be benefit to the hospital unit involved, giving a better structure to the stages of this methodology of humanization, knowing the meaning of being a puerperal woman in KMC, beyond to contribute to amplify the reflection of the academic and health communities related to the way of implementation of the Method. I bring out the importance of this Method to the child health and to the increase of the familiar link. However, we have consider that for the full success, the experience of the woman in the method has to be highly valued.
