Título: Causas de interrupção da nutrição enteral em Unidades de Terapia Intensiva

Abstract: Introdução: O suprimento nutricional adequado é importante para garantir melhores desfechos clínicos ao paciente crítico, porém, observa-se que a Nutrição Enteral (NE) está associada à subalimentação devido a interrupções que influenciam na oferta plena da dieta. O objetivo deste estudo foi identificar as causas da interrupção da nutrição enteral de pacientes internados em Unidades de Terapia Intensiva (UTI). Método: Estudo de coorte, prospectivo, realizado com pacientes internados nas UTIs de um Hospital Geral do Sudoeste da Bahia, no período de Março a Dezembro de 2018. Foram coletados dados epidemiológicos, clínicos e informações quanto ao volume de infusão da dieta, fatores e tempo de interrupção da nutrição enteral no prontuário e ficha de controle padronizada na unidade. Os dados foram coletados até o sétimo dia de internação e acompanhados até desfecho clínico (alta ou óbito). Os dados foram analisados com o programa SPSS 21.0 por meio da estatística descritiva. Resultados: Foram obtidos dados de 139 pacientes. As principais causas de interrupção da NE foram falha na administração (42,9%), jejum para procedimentos (30,0%) e resíduo gástrico com volume menor ou maior que 500 ml/24h (29,3%). O tempo médio em horas por dia de interrupção da nutrição enteral foi 11,2±7,14 e a média de dias que houve interrupção 3,0 ± 1,89 por paciente. A média do volume prescrito foi 1.113±389 mL enquanto a média do volume infundido foi 719±303 mL com apenas 64% da dieta prescrita administrada ao paciente. Conclusão: Observou que as interrupções da NE ocorrem com frequência nas UTIs pesquisadas. Ressalta-se que o tempo de interrupção pelas principais causas são potencialmente controláveis e, portanto, os esforços devem ser direcionados à implementação de estratégias com toda a equipe multiprofissional para minimizar os danos da subalimentação diante da administração parcial da dieta prescrita.
Introduction: Adequate nutritional supply is important to ensure better clinical outcomes for critically ill patients. However, it is observed that Enteral Nutrition (NE) is associated with malnutrition due to interruptions that influence the full diet supply. The objective of this study was to identify the causes of interruption of enteral nutrition of patients hospitalized in Intensive Care Units (ICU). Method: A prospective cohort study with patients admitted to the ICUs of a General Hospital of the Southwest of Bahia from March to December, 2018. Epidemiological and clinical data and information on the volume of infusion of the diet, time of interruption of enteral nutrition in the medical record and standardized control card in the unit. Data were collected until the seventh day of hospitalization and followed up until clinical outcome (discharge or death). Data were analyzed with the SPSS 21.0 program using descriptive statistics. Results: Data were obtained from 139 patients. The main causes of interruption of NE were failure to administer (42.9%), fasting for procedures (30.0%) and gastric residue with a volume of less than or greater than 500 ml / 24h (29.3%). The mean time in hours per day of interruption of enteral nutrition was 11.2 ± 7.14 and the mean number of days that there was interruption was 3.0 ± 1.89 per patient. The mean volume prescribed was 1,113 ± 389 mL while the mean volume infused was 719 ± 303 mL with only 64% of the prescribed diet administered to the patient. Conclusion: It was observed that interruptions of NE occur frequently in the ICUs studied. It should be pointed out that the interruption time for the main causes are potentially controllable and therefore, efforts should be directed to the implementation of strategies with the whole multiprofessional team to minimize the damages of the malnutrition before the partial administration of the prescribed diet.
