dc.description.abstractObesity is associated with worse clinical outcomes in cutaneous leishmaniasis (CL). Here, we evaluated the influence of obesity on the systemic, dermal, and adipose tissue immune responses in CL. A total of 39 obese and 43 individuals with normal BMI with ulcerated CL caused by Leishmania braziliensis confirmed by PCR were enrolled, along with 8 healthy subjects (HS), 4 with normal BMI and 4 obese. Peripheral blood mononuclear cells (PBMC) were cultured for 72 hours in the presence or absence of soluble Leishmania antigen (SLA, 5 μg/mL). Skin and adipose tissue biopsies were analyzed histologically, and cytokines and adipokines were quantified after 72-hour incubation. All patients were treated with meglumine antimoniate. The cure rate was lower in obese patients (41%) compared to those with normal BMI (70.4%) (p < 0.05). No significant differences in proinflammatory cytokine production were observed in PBMC supernatants. However, cytokines (IL-1β, IL-17, Granzyme B) and leptin were significantly higher in skin biopsies of obese compared to normal BMI CL patients (p < 0.05), and leptin levels correlated directly with these cytokines in both groups. Adipose tissue was infiltrated by inflammatory cells, with larger inflammatory areas in obese CL patients (p < 0.05). Cytokines were largely undetectable in adipose tissue, except for elevated CCL2. Notably, leptin and adiponectin levels were markedly increased in CL patients compared to HS, suggesting infection-driven modulation of adipocyte function. These findings demonstrate that CL enhances adipokine production and that obesity amplifies local inflammatory responses, contributing to greater disease severity and therapeutic failure in obese individuals.pt_BR

dc.description.resumoA leishmaniose cutânea (LC) é uma doença infecciosa causada por Leishmania braziliensis que apresenta maior frequência de falha terapêutica em indivíduos obesos, porém os mecanismos que relacionam obesidade e pior desfecho clínico ainda não estão completamente esclarecidos. OBJETIVO: Avaliar a influência da obesidade na resposta imune sistêmica, cutânea e do tecido adiposo em pacientes com LC. MÉTODOS: Foram incluídos 39 indivíduos obesos e 43 indivíduos com IMC normal com LC ativa confirmada por PCR, além de 8 controles sem LC (4 obesos e 4 com IMC normal). Células mononucleares de sangue periférico (CMSP) foram cultivadas com antígeno solúvel de Leishmania (SLA), 5 µg/mL, por 72 horas. Biópsias de pele e tecido adiposo foram submetidas à análise histopatológica e dosagens de citocinas e adipocinas após 72 horas de incubação. RESULTADOS: A taxa de cura no D90 foi menor nos obesos (41%) quando comparada aos indivíduos com IMC normal (70%; p < 0,05). Não foram observadas diferenças significativas na produção de citocinas por CMSP. Entretanto, em biópsias de pele, IL-1β, IL-17, granzima B e leptina foram detectadas em concentrações mais elevadas em obesos (p < 0,05), com forte correlação direta entre leptina e essas citocinas. O tecido adiposo apresentou infiltrado inflamatório mais extenso em obesos (p < 0,05), embora com baixa produção local de citocinas, exceto CCL2. Leptina e adiponectina encontraram-se aumentadas em pacientes com LC em relação a controles saudáveis, sugerindo modulação da atividade adipocitária pela infecção. CONCLUSÕES: A obesidade intensifica a resposta inflamatória local na LC, favorecendo mecanismos associados à gravidade da doença e contribuindo para a maior taxa de falha terapêutica observada em indivíduos obesos.pt_BR
