| dc.description.abstract | Congenital cataract is a major cause of reversible childhood blindness, and although surgical treatment is essential, it may lead to significant complications, including glaucoma. This cross-sectional, retrospective, observational study evaluated the clinical characteristics of patients who developed glaucoma after congenital cataract surgery at the Pediatric Ophthalmology Service of HUPES-UFBA between July 2017 and June 2023. Among 145 operated patients (225 eyes), 4.8% developed postoperative glaucoma (10 eyes). Most affected patients underwent surgery between 4 and 6 months of age, a period associated with increased glaucoma risk. Systemic comorbidities—especially Down syndrome—were present in 57% of glaucoma cases. All patients underwent lensectomy with posterior capsulotomy and anterior vitrectomy, without intraocular lens implantation. Glaucoma was diagnosed on average 37 months after surgery, with a mean intraocular pressure of 26.4 mmHg and a mean optic disc cupping of 0.65. Multiple interventions were often required for pressure control, including combined medical and surgical therapies. All patients showed deterioration of visual acuity after glaucoma onset. The findings highlight the importance of lifelong follow-up for children undergoing congenital cataract surgery, especially those operated at an early age or with systemic comorbidities, and emphasize the need for structured protocols for early glaucoma detection and timely intervention | pt_BR |

| dc.description.resumo | A catarata congênita é uma importante causa de cegueira infantil reversível, cujo tratamento cirúrgico, apesar de necessário, pode evoluir com complicações relevantes, entre elas o glaucoma. Este estudo transversal, retrospectivo e observacional analisou as características clínicas dos pacientes submetidos à cirurgia de catarata congênita no setor de Oftalmopediatria do HUPES-UFBA entre julho de 2017 e junho de 2023, com foco naqueles que desenvolveram glaucoma pós-operatório. Dos 145 pacientes operados (225 olhos), 4,8% evoluíram com glaucoma (10 olhos). A maioria foi operada entre 4 e 6 meses de idade, faixa etária associada a maior risco da complicação. Comorbidades sistêmicas, especialmente a Síndrome de Down, foram frequentes entre os pacientes com glaucoma (57%). Todos foram submetidos à facectomia com capsulotomia posterior e vitrectomia anterior, sem implante de lente intraocular. O diagnóstico de glaucoma ocorreu, em média, 37 meses após a cirurgia, com pressão intraocular média de 26,4 mmHg e escavação papilar média de 0,65. A maior parte dos pacientes necessitou de terapias combinadas, incluindo tratamentos cirúrgicos. Observou-se piora universal da acuidade visual após o desenvolvimento do glaucoma. O estudo reforça a necessidade de acompanhamento vitalício desses pacientes, especialmente naqueles operados precocemente e/ou com comorbidades sistêmicas, além da implementação de protocolos estruturados de detecção precoce do glaucoma. | pt_BR |
