dc.description.abstractIntroduction: Several illness-specific cultural and system-based barriers to palliative care (PC) integration and optimal end-of-life (EOL) care exist in the oncohematology scenario. Limited research exists exploring determinants of the discrepancies in daily implementation of PC among healthcare professionals in this field. Objective: We aimed to investigate the variability in PC and EOL care among hematologists and hematology residents at the hematology service of Clinics Hospital of the Federal University of Bahia (UFBA). Methods: Cross-sectional study performed between October and December 2022. Variables were collected through a sociodemographic questionnaire and an adaptation of clinical questionnaires used in previous studies. Results: During the research period, all twenty physicians from the hematology division of the Clinics Hospital of the UFBA participated. Median age was 44 years-old, with 80% of participants identifying as female and 75% were graduated hematologists. Participants were presented with a hypothetical scenario involving the treatment of a 65-year-old female patient with poor prognosis acute myeloid leukemia refractory to first-line treatment. Sixty percent of participants chose to follow other chemotherapy regimens, while 40% implemented PC. Participants next considered a case salvage for the patient who developed septic shock following chemotherapy and were prompted to choose their most probable conduct and the conduct they thought would be better for the patient. Even though being part of the same center, we found a divergence from the most probable conduct among 40% of participants. Reasons involved personal convictions, legal aspects and other physicians’ reactions. Discussion: We found considerable differences within PC decisions between professionals despite being under the same service and protocols. Such investigations hold immense importance for existing literature, as they may identify modifiable factors that can effectively enhance quality of PC and EOL care within the oncohematology setting.pt_BR

dc.description.resumoIntrodução: No cenário da oncohematologia, existem diversas barreiras culturais e específicas das doenças para a adequada integração de cuidados paliativos (CP) e cuidados de fim de vida (CFV). Ademais, há poucas pesquisas na literatura que exploram os determinantes das discrepâncias na implementação diária de CP entre os profissionais de saúde neste campo. Objetivo: Investigar a variabilidade em CP e CFV entre hematologistas e residentes de hematologia do serviço de hematologia do Hospital das Clínicas da Universidade Federal da Bahia (UFBA). Métodos: Estudo de corte transversal realizado entre outubro e dezembro de 2022. As variáveis foram coletadas através de um questionário sociodemográfico e da adaptação de questionários clínicos utilizados em estudos anteriores. Resultados: No período da pesquisa, participaram todos os vinte médicos do serviço de hematologia do Hospital das Clínicas da UFBA. A média de idade foi de 44 anos, com 80% dos participantes do sexo feminino e 75% de hematologistas graduados. Aos participantes, foi apresentado um cenário hipotético envolvendo o tratamento de uma paciente de 65 anos com leucemia mieloide aguda de mau prognóstico, refratária ao tratamento de primeira linha. Sessenta por cento dos participantes optaram por seguir outros esquemas de quimioterapia, enquanto 40% implementaram CP. Em seguida, foi apresentada evolução do caso, em que a paciente desenvolveu choque séptico após instituição de quimioterapia e os participantes foram solicitados a escolher sua conduta mais provável e a conduta que acreditavam que seria melhor para a paciente. Apesar de fazerem parte do mesmo serviço, foi encontrada divergência entre as condutas entre 40% dos participantes. Razões envolviam convicções pessoais, aspectos legais e reações de outros médicos. Discussão: Encontramos diferenças consideráveis nas decisões de CP entre os profissionais, apesar de estarem sob as orientações de um mesmo serviço e sob os mesmos protocolos. Tais investigações apresentam imensa importância para a literatura existente, visto que podem identificar fatores modificáveis que podem efetivamente melhorar a qualidade em CP e CFV no contexto da oncohematologia.pt_BR
