dc.description.abstractObstructive Sleep Apnea (OSA) is common among patients with cardiovascular and cerebrovascular events and is being considered an independent risk factor for stroke. Despite this, the scientific understanding and documentation of this pathophysiological relationship is still under development. Transcranial Doppler is a non-invasive method of assessing cerebral blood flow and its possible changes. BHI (breath-holding index) is a method of assessing cerebral vasoreactivity, understood as the response of cerebral blood vessels to changes in the partial pressure of CO2 (carbon dioxide) and O2 (oxygen), through transcranial Doppler. Cerebral vasoreactivity is considered an adaptation to metabolic changes in cerebral vessels, and its impairment is a marker of stroke risk. Some studies have documented changes in cerebral vasoreactivity in patients with an increased risk of OSA or with OSA confirmed by polysomnography, in the general population. Cerebral vasoreactivity in patients with stroke and OSA confirmed by polysomnography has not yet been studied and may contribute to better understanding of the pathophysiology of cerebrovascular disease in the OSA population, allowing planning of possible therapeutic interventions. Aimed to identify a possible association between cerebral vasoreactivity - measured by BHI during transcranial Doppler - and the severity of OSA - measured by the AHI (Apnea Hypopnea Index) recorded in type 2 sleep monitoring (portable polysomnography), was evaluated a group of 83 patients with a confirmed diagnosis of stroke; aged over 18 years; with increased AHI (>5/h) in type 2 sleep monitoring; underwent assessment of cerebral vasoreactivity by transcranial Doppler, through BHI; who agreed to participate in the study through the Informed Consent Form (ICF). The mean age of the patients studied was 60.2 years, of which 62 (74.7%) were male. 48 (57.8%) patients had mild to moderate OSA (AHI<30/h) and 38 (45.8%) had altered cerebral vasoreactivity (BHI<0.69). A negative correlation was observed between the cerebral vasoreactivity index and the severity of OSA specifically during non-REM sleep (NREM sleep), with Spearman correlation = -0.23, p=0.036. In the linear regression adjusted for age and sex, however, this association was not maintained (effect = -0.01; 95% CI = -0.01 to 0.002, p=0.134). In the present study, therefore, changes in cerebral vasoreactivity were associated with OSA severity specifically during NREM sleep.pt_BR

dc.description.resumoA Apneia Obstrutiva do Sono (AOS) é uma condição comum entre pessoas que sofreram eventos cardiovasculares e cerebrovasculares, considerada um fator de risco independente para o Acidente Vascular Cerebral (AVC). Apesar de diversos autores já defenderem essa associação entre AVC e AOS, a compreensão e documentação científica dessa relação fisiopatológica ainda está em construção. O doppler transcraniano é um método não invasivo de avaliação do fluxo sanguíneo cerebral e suas possíveis alterações. O BHI (breath-holding index) é um método de avaliação da vasorreatividade cerebral, compreendida como resposta dos vasos sanguíneos cerebrais às alterações na pressão parcial de CO2 (gás carbônico) e O2 (oxigênio), através do doppler transcraniano. A vasorreatividade cerebral é considerada uma adaptação às alterações metabólicas dos vasos cerebrais e seu prejuízo é um marcador de risco para o AVC. Alguns estudos documentaram alteração da vasorreatividade cerebral em pacientes com risco aumentado de AOS ou com AOS confirmada por polissonografia, na população em geral. A vasorreatividade cerebral em pacientes com AVC e AOS confirmada por polissonografia ainda não foi estudada e pode contribuir para melhor compreensão da fisiopatologia da doença cerebrovascular na população com AOS e para melhor estratificação do risco cerebrovascular desses pacientes, permitindo planejamento de possíveis intervenções terapêuticas. Com o objetivo identificar possível associação entre vasorreatividade cerebral - mensurada pelo BHI durante realização de doppler transcraniano - e a gravidade da AOS - mensurada pelo IAH (Índice de Apneia e Hipopneia) registrado na monitorização tipo 2 do sono, foi avaliado um grupo de 83 pacientes com o diagnóstico confirmado de AVC; com idade maior de 18 anos; com IAH aumentado (>5/h) em exame de monitorização tipo 2 do sono; submetidos à avaliação da vasorreatividade cerebral pelo doppler transcraniano, através do BHI; que concordaram em participar do estudo através do Termo de Consentimento Livre Esclarecido (TCLE). A idade média dos pacientes estudados foi 60,2 anos, sendo 62 (74,7%) pacientes do sexo masculino. 48 (57,8%) pacientes tinham AOS de intensidade leve a moderada (IAH<30/h) e 38 (45,8%) tinham alteração da vasorreatividade cerebral (BHI< 0,69). Foi observada uma correlação negativa entre o índice de vasorreatividade cerebral e a gravidade da AOS especificamente durante o sono sem movimentos rápidos dos olhos (sono NREM), com correlação de Spearman = -0,23, p=0,036. Na regressão linear ajustada para idade e sexo, no entanto, essa associação não se manteve (efeito = -0,01; IC 95% = -0,01 a 0,002, p=0,134). No presente estudo, portanto, a alteração da vasorreatividade cerebral esteve associada à gravidade da AOS especificamente durante o sono não REM.pt_BR
