dc.description.abstractIntroduction: Traumatic brain injury can result in oculomotor and vestibular dysfunctions, impacting balance, gaze stabilization, and sensory integration, often associated with symptoms of dizziness. Different pathophysiological mechanisms have been proposed to explain the coexistence of these alterations, however, the detailed characterization of these deficits is not yet consolidated. Objective: To describe characteristics of oculomotor dysfunctions and complaints of dizziness in individuals with traumatic brain injury. Method: This is a systematic review of the literature, conducted according to the PRISMA checklist and registered in PROSPERO (CRD42021289745). The search was performed in the PubMed and BVS databases, using MeSH descriptors. Analytical observational studies that evaluated eye movements and dizziness in individuals with traumatic brain injury were selected. Studies with animals, use of medications, samples smaller than ten participants, and non-indexed publications were excluded. Methodological quality and risk of bias were assessed using tools from the Joanna Briggs Institute. Results: The survey resulted in 11 articles, of which seven were included. These were published between 2015 and 2024 and involved 460 individuals, mostly men (18-70 years old) with concussion and traumatic brain injury. Dizziness was assessed using the Dizziness Handicap Inventory in all studies, with scores being significantly higher in individuals with traumatic brain injury. Deficits in saccades, smooth pursuit, and vestibulo-ocular reflex were identified, in addition to alterations in tests such as nystagmus, vertical and torsional alignment nullification, Gaze Stabilization Test, and ocular vestibular evoked myogenic potential. Some studies have described a relationship between the Dizziness Handicap Inventory and a greater number of oculomotor dysfunctions. Conclusion: The results of this review reinforce that individuals with traumatic brain injury, in addition to complaining of persistent dizziness, often present oculomotor dysfunctions, suggesting that the integrity of the oculomotor system is frequently compromised in these cases.pt_BR

dc.description.resumoIntrodução: A lesão cerebral traumática pode resultar em disfunções oculomotoras e vestibulares, impactando o equilíbrio, a estabilização do olhar e a integração sensorial, frequentemente associadas à sintomas de tontura. Diferentes mecanismos fisiopatológicos foram propostos para explicar a coexistência dessas alterações, porém, a caracterização detalhada desses déficits ainda não está consolidada. Objetivo: Descrever características de disfunções oculomotoras e queixa de tontura em indivíduos com lesão cerebral traumática. Método: Trata-se de uma revisão sistemática da literatura, conduzida de acordo com o checklist PRISMA e registrada no PROSPERO (CRD42021289745). A busca foi realizada nas bases PubMed e BVS, utilizando descritores do MeSH. Foram selecionados estudos observacionais analíticos que avaliaram movimentos oculares e tontura em indivíduos com lesão cerebral traumática. Excluíram-se estudos com animais, uso de medicamentos, amostras menores que dez participantes e publicações não indexadas. A qualidade metodológica e o risco de viés foram avaliados com ferramentas do Joanna Briggs Institute. Resultados: O levantamentou resultou em 11 artigos, dos quais sete foram incluídos. Estes foram publicados entre 2015 e 2024, envolveram 460 indivíduos, majoritariamente homens (18-70 anos) com concussão e traumatismo cranioencefálico. A avaliação da tontura ocorreu utilizando o Dizziness Handicap Inventory em todos os estudos, sendo que as pontuações foram significativamente mais elevadas em inivíduos com lesão cerebral traumática. Foram identificados déficits em sacadas, perseguição suave e reflexo vestíbulo-ocular, além de alterações em testes como nistagmo, anulação de alinhamento vertical e torcional, Gaze Stabilization Test e potencial miogênico evocado vestibular ocular. Alguns estudos descreveram uma relação entre o Dizziness Handicap Inventory e maior número de disfunções oculomotoras. Conclusão: Os resultados da presente revisão reforçam que indivíduos com lesão cerebral traumática, além de se queixarem de tontura persistente, frequentemente apresentam disfunções oculomotoras, sugerindo que a integridade do sistema oculomotor é frequentemente comprometida nesses casos.pt_BR
