| dc.description.abstract | Maintaining newborns’ (NB) thermal stability is a challenge for health professionals and
requires accurate practice, as the performance of invasive and surgical procedures predisposes
these patients to hypothermia. It is the most reported adverse event in neonatal units. The
general objective was to build and validate a protocol for the prevention of hypothermia in
NBs in the perioperative period; the specific objectives were: to identify the strategies that
have been used to prevent hypothermia in NBs in the perioperative period; identify the factors
that predispose to hypothermia in NBs in the perioperative period, from the perspective of
health professionals who are part of the care team; describe care strategies for the prevention
of hypothermia in NBs in the perioperative period; and perform content and structure
validation of a protocol for the prevention of hypothermia in NBs in the perioperative period.
The literature review addressed interventions to control hypothermia in NBs. The theoretical
framework cuts across: thermoregulation of surgical NBs; patient safety when approaching
surgical NBs; and environmental theory anchoring neutral thermal care to NBs. As a
methodological framework, Convergent Care Research (PCA) was chosen, with the setting of
a private maternity hospital in the capital of Bahia, developed in two phases: in the first,
construction of the protocol with the triangulation of data from the theoretical framework,
literature review, observation, interviews and a convergence group, with 21 health
professionals as participants; in the second phase, content validation was carried out, guided
by the Delphi method, comprising 12 judges. The interpretation of qualitative data was guided
by the analysis model proposed by PCA, and in the analysis of quantitative data, the
individual and general content validation index was used. The results show that, among the
strategies identified in the scientific literature for the prevention of hypothermia, the following
stand out: room temperature control; humidification establishment and cleaning quality of the
air conditioning; use of an incubator or heated crib; use of thermal mattress; use of caps and
blankets; heated fluids; temperature monitoring; coverage of abdominal organs. Factors that
predispose to hypothermia are: immaturity of the thermoregulatory system; temperature of the
surgical environment; intra-hospital transport; cold liquids; inadequacy in the use of
technological resources; non-recognition of hypothermia as a health problem; and failure in
continuing education. With data triangulation, a protocol was built and divided into three
operative periods, pre, intra and post; each period consisted of five domains, 48 objectives
with justification, and 85 care strategies. In validation, a general Content Validation Index of
0.94 was obtained. It can be concluded that the protocol proved to be a novel, judicious, valid
and reliable instrument for use in the prevention of hypothermia in NBs in the perioperative
period. It is believed that this technology makes it possible to reduce risks, injuries and
damage to health, as well as to qualify assistance, based on patient safety policy and
environmentalist strategies implemented by Nightingale. In this way, it will make it possible
to improve the care provided to NBs’ health, as it can redirect assistance actions and
contribute to the reduction of neonatal morbidity and mortality. | pt_BR |

| dc.description.resumo | Manter a estabilidade térmica do recém-nascido (RN) é um desafio para os profissionais de
saúde e requer uma prática acurada, visto que a realização de procedimentos invasivos e
cirúrgicos predispõe à hipotermia nesses pacientes. Configura-se como o evento adverso de
maior notificação em unidades neonatais. O objetivo geral foi construir e validar um
protocolo para prevenção de hipotermia em RN no período perioperatótio; os objetivos
específicos foram: identificar as estratégias que vêm sendo utilizadas para prevenção de
hipotermia em RN no período perioperatório; identificar os fatores que predispõem à
hipotermia em RN no período perioperatório, na perspectiva dos profissionais de saúde que
integram a equipe assistencial; descrever estratégias de cuidado para prevenção de
hipotermia em RN no período perioperatório; e realizar a validação de conteúdo e da
estrutura de um protocolo para prevenção de hipotermia em RN no período perioperatório. A
revisão de literatura abordou as intervenções para controle da hipotermia no RN. O
referencial teórico transversaliza entre: termorregulação do RN cirúrgico; segurança do
paciente na abordagem ao RN cirúrgico; e teoria ambientalista ancorando o cuidado térmico
neutro ao RN. Como referencial metodológico, optou-se pela Pesquisa Convergente
Assistencial (PCA), tendo como cenário uma maternidade privada da capital baiana,
desenvolvida em duas fases: na primeira, construção do protocolo com a triangulação dos
dados do referencial teórico, revisão de literatura, observação, entrevistas e grupo de
convergência, tendo como participantes 21 profissionais de saúde; na segunda fase,
procedeu-se a validação de conteúdo, guiada pelo método Delphi, sendo composta por 12
juízes. A interpretação dos dados qualitativos foi guiada pelo modelo de análise proposta
pela PCA, e, na análise dos dados quantitativos, foi utilizado o índice de validação de
conteúdo individual e geral. Os resultados constatam que, dentre as estratégias identificadas
na literatura científica para prevenção da hipotermia, destacam-se: controle da temperatura
ambiente; estabelecimento de umidificação e qualidade de limpeza do ar condicionado;
utilização de incubadora ou berço aquecido; uso de colchão térmico; uso de toucas e
cobertor; fluidos aquecidos; monitoramento da temperatura; cobertura de órgãos abdominais.
Os fatores que predispõem à hipotermia são: imaturidade do sistema termorregulador;
temperatura do ambiente cirúrgico; transporte intrahospitalar; líquidos frios; inadequação na
utilização dos recursos tecnológicos; não reconhecimento da hipotermia como agravo; e
falha na educação continuada. Com a triangulação dos dados, foi construído um protocolo
que foi dividido em três períodos operatórios, pré, intra e pós; cada período foi composto por
cinco domínios, 48 objetivos com justificativa e 85 estratégias de cuidado. Na validação,
obteve-se Índice de Validação de Conteúdo geral de 0,94. Pode-se concluir que o protocolo
se mostrou um instrumento inédito, criterioso, válido e confiável para uso na prevenção da
hipotermia do RN em periodo perioperatório. Acredita-se que essa tecnologia permite
reduzir os riscos, agravos e danos à saúde, bem como qualificar a assistência, tendo como
norte a política de segurança do paciente e as estratégias ambientalistas trazidas por
Nightingale. Dessa forma, possibilitará melhorar o cuidado prestado à saúde do neonato,
visto que pode redirecionar ações assistenciais e contribuir com a redução da
