Política de saúde para a população LGBT: reflexões e aproximações da gênese no estado da Bahia

Abstract

Os direitos sociais, em especial, à saúde é uma das reinvindicações mais antigas da população de Lésbicas, Gays, Bissexuais, Travestis e Transexuais (LGBT) em diversos países e no Brasil. Ainda que exista um debate nacional sobre o processo de construção de pautas relacionadas à políticas de saúde, a partir de demandas derivadas dos movimentos sociais, este ainda é incipiente no Estado da Bahia. Essa dissertação analisou a gênese de ações relacionadas à política nacional de Saúde para as Lésbicas, Gays, Bissexuais, Travestis e Transexuais (LGBT), por meio de um estudo de sociogênese no período compreendido entre 1979 e 2014. A análise se fundamentou teoricamente em estudos de revisão e em reflexões subsidiadas pela sociologia reflexiva de Bourdieu, apoiado na proposta de Pinell para a análise sociológica de políticas públicas. Os resultados foram estruturados em quatro artigos. O primeiro artigo analisou a conformação de políticas de saúde para a população LGBT em alguns contextos internacionais. O segundo revisou o estado da arte sobre a inclusão da discussão sobre a atenção à saúde a população LGBT nos currículos internacionais dos cursos de graduação na área da saúde para compreender vazios assistenciais históricos. O terceiro artigo, que constitui um capítulo de livro, contemplou uma revisão documental sobre a gênese da Política Nacional de Saúde Integral de Lésbicas, Gays, Bissexuais, Travestis e Transexuais (PNSI-LGBT) no Brasil e permitiu identificar os movimentos sociais nacionais e as bifurcações para o Estado da Bahia. Essas produções forneceram elementos para a compreensão do objeto de estudo central desvelado pelo quarto artigo. Neste, o levantamento bibliográfico e organização temporal dos fatos históricos permitiram reconstruir a dinâmica do espaço social e da participação dos agentes dos campos burocrático, científico e político, bem como do espaço militante (ou associativo) para a gênese de ações relacionadas à política nacional de saúde LGBT no estado da Bahia. A análise do espaço social foi mediada pelo estudo das trajetórias dos agentes envolvidos com a formulação da política no estado da Bahia e as relações entre esses agentes e o espaço social nacional. As condições de possibilidade históricas que permitiram a formulação de ações relacionadas a política nacional foram a publicação do Plano Bahia sem homofobia, que permitiu ampliar o diálogo com a sociedade civil e os movimentos sociais e abarcar as principais críticas para a formulação de ações vinculadas a PNSI-LGBT como resposta assistencial às reais necessidades em saúde da população, a formalização da área técnica LGBT na SESAB em 2013 e a instituição do Comitê Técnico Estadual de Saúde Integral LGBT da Bahia, em 2014, que se desdobraram em ações direcionadas à garantia de maior integralidade e equidade da atenção à saúde.

Social rights, especially health rights, is one of the oldest claims of the lesbian, gay, bisexual, transvestite and transgender (LGBT) population in several countries and in Brazil. The debate on the process of building agendas related to health policies, based on demands derived from social movements, has not yet deepened. This dissertation analyzed the genesis of the state health policy for Lesbians, Gays, Bisexuals, Transvestites and Transsexuals (LGBT), through a sociogenesis study in the period from 1979 to 2014. The analysis was theoretically based on Bourdieu's reflexive sociology, supported by Pinell's proposal for the sociological analysis of public policies. The results were structured in four articles. The first article analyzed the conformation of health policies for the LGBT population in some international contexts. The second article reviewed the state of the art on the inclusion of the discussion on health care for the LGBT population in the international curricula of undergraduate health courses to understand historical care gaps. The third article, which constitutes a book chapter, contemplated a document review on the genesis of the National Policy for Integral Health of Lesbians, Gays, Bisexuals, Transvestites and Transsexuals (PNSI-LGBT) in Brazil and allowed to identify national social movements and bifurcations for the State of Bahia. These productions provided elements for the understanding of the central study object unveiled by the fourth article. In this article, the bibliographic survey and temporal organization of historical facts allowed for reconstructing the dynamics of social space and participation of agents from bureaucratic, scientific and political fields, as well as the militant (or associative) space for the genesis of actions related to the national LGBT health policy in the state of Bahia. The analysis of the social space was mediated by the study on the trajectories of agents involved with policy formulation in the State of Bahia and the relationships between these agents and the national social space. The historical conditions of possibility that allowed the formulation of actions related to national policy were the publication of the Bahia Plan without homophobia, which allowed to expand the dialogue with civil society and social movements and encompass the main criticisms for the formulation of actions linked to PNSI -LGBT as an assistance response to the real health needs of the population, the formalization of the LGBT technical area at SESAB in 2013 and the institution of the State Technical Committee for Integral Health LGBT in Bahia, in 2014, which unfolded in actions aimed at ensuring greater integrality and equity of health care.
