The HTLV-1 is the first retrovirus identified with oncogenic potential in humans and has well-documented relationship with the adult T-cell lymphoma (ATL) pathogenesis. One of the endemic countries for this disease is Brazil and the highest number of cases are described in the north and northeast regions. The clinical progression occurs after a long latency period , and cutaneous lesions are common present in the clinical presentation. They can usually be the first manifestation in asymptomatic individuals. This case describes cutaneous ATL involvement in a middle-aged female patient over the course of two years until the time of diagnostic confirmation and with characteristics that diverge from the expected findings described in the literature published in highly relevant medical databases.
