dc.description.abstract Sickle Cell Disease (SCD)is a genetic condition that primarily affects individuals of African descent, caused by a mutation in hemoglobin that distorts red blood cells into a sickle shape. Predominant in regions with a history of slavery, such as Brazil, SCD leads to serious complications like anemia and osteonecrosis. Early diagnosis is essential for effective interventions, and new therapeutic approaches, such as cell therapy, offer promising treatment alternatives. Objective: This study describes the progression of bone regeneration in shoulder osteonecrosis in patients with sickle cell disease treated with bone marrow- derived mesenchymal cell implants, assessing the degree of biomechanical recovery. Methodology: A non-randomized clinicaltrial was conductedwith adults diagnosed with humeral head osteonecrosis, treated at the Professor Edgard Santos University Hospital Complex (COM-HUPES). Patients used a sling for two days post-treatment, followed by active shoulder mobilization. Information on comorbidities, BMI, and demographic data was collected, along with pre- and post-operative X-ray and MRI exams, repeated12 months aftersurgery. Follow- up visits occurred one week prior to the procedure and at 3 weeks, 3 months, 6 months, and 12 months post-surgery, with evaluations conducted by the orthopedic team.Results: Comparison of pre- and post-operative range of motion showed improvements in flexion, extension, internal and external rotation. Clinical assessment indicated reducedpain and disability. Comparisons between pre- and post-operative MRIs revealeda decrease in bone edemaand the extent of osteonecrosis. No patients showed deformity of the humeral head or signs of osteoarthritis. Conclusion: The therapy using autologous bone marrow mononuclear cell implants demonstrated effectiveness in improving range of motion and SPADI scores in patients with humeral head osteonecrosis secondary to sickle cell disease. There were visible improvements in MRI scans, with reductions in bone edema and necrosis extent after one year, and the treatment proved to be safe with no post-operative complications. pt_BR

dc.description.resumo A Doença Falciforme (DF), é uma condição genética que afeta principalmente pessoas de ascendência africana, causada por uma mutação na hemoglobina que distorceos glóbulos vermelhosem forma de foice. Predominante em regiões com histórico de escravidão, como o Brasil, a DF gera complicações graves, como anemia e osteonecrose. O diagnóstico precoce é essencial para intervenções eficazes, e novas abordagens terapêuticas, como a terapia celular, oferecem promissoras alternativas de tratamento. Objetivo: Este estudo descreve a evolução da regeneração óssea da osteonecrose do ombro em pacientes com doença falciforme tratados com implante de células mesenquimaisda medula óssea,avaliando o grau de recuperação biomecânica. Metodologia: Foi realizado um ensaio clínico não randomizado com adultos, diagnosticados com osteonecrose da cabeça umeral, tratados no Complexo Hospitalar Universitário Professor Edgard Santos (COM-HUPES). Os pacientes utilizaram tipoia por dois dias após o tratamento e, em seguida, iniciaram mobilização ativa do ombro. Informações sobre comorbidades, IMC e dados demográficos foram coletadas, além de exames iniciais de radiografia e ressonância magnética (RM) pré e pós-operatória, repetidos 12 meses após a cirurgia. As visitas de acompanhamento ocorreram uma semana antes do procedimento, e com 3 semanas, 3 meses, 6 meses e 12 meses após, sendo avaliados pela equipe de ortopedia. Resultados: A comparação da amplitude de movimentos pré e pós-operatória mostrou melhora em flexão, extensão,rotação interna e externa. A avaliação clínica, indicou redução da dor e incapacidade. Comparações entre RMs pré e pós-operatórias revelaram redução do edema ósseo e da extensão da osteonecrose. Nenhum paciente apresentou deformidade da cabeça umeral ou sinais de osteoartrite. Conclusão: A terapia com implante autólogo de célulasmononucleares da medula óssea demonstrou eficácia na melhora da amplitude de movimento e das pontuações do SPADI em pacientes com osteonecrose da cabeça umeralsecundária à doença falciforme. Houve melhorias visíveis nos exames de RM, com redução do edema ósseo e da extensão da necrose após um ano, além de ser um tratamento seguro, sem complicações pós-operatórias. pt_BR
