Título: LÍQUEN PLANO ORAL: ASSOCIAÇÃO ENTRE LESÕES BRANCAS E VERMELHAS E CARACTERÍSTICAS HISTOMORFOMÉTRICAS

Abstract: Introdução: O líquen plano oral (LPO) é considerado uma desordem oral potencialmente maligna com possibilidade de manifestações clínicas variadas, caracterizado histologicamente pela presença de uma faixa de infiltrado inflamatório subepitelial e pela degeneração da camada basal do epitélio. A presença da camada de ceratina e dos corpos de Civatte também podem ser identificadas. O diagnóstico do LPO é clínicopatológico, o que pode gerar subjetividade. Hipotetiza-se que o uso da histomorfometria poderia contribuir para uma análise mais objetiva para o diagnóstico destas lesões. Objetivo: Verificar a associação entre os grupos de lesões brancas e vermelhas e diferenças histomorfométricas nas lesões de LPO. Métodos: Tratou-se de um estudo retrospectivo transversal, onde foram analisados 48 laudos e cortes histológicos, que pertenciam ao acervo de dois laboratórios distintos de Instituições de Ensino Superior, e que foram submetidas a avaliações sob o ponto de vista de variáveis morfológicas e medidas morfométricas. As análises histomorfométricas compreenderam a mensuração entre a distância basal à superfície epitelial, a distância do ápice da crista epitelial à superfície epitelial, distância entre as cristas epiteliais, largura da crista epitelial, espessura da camada de ceratina, espessura da faixa subepitelial do infiltrado inflamatório, extensão e profundidade da úlcera. Resultados: Foi observado que 81,2% da amostra era do sexo feminino e 50,0% tinha menos de 50 anos. Quanto ao local da lesão observou-se que em 58,3% dos casos a mesma ocorreu na mucosa jugal, sendo que 83,3% da amostra apresentaram lesão branca e 18,8% eram fumantes. Notou-se que 10,4%, 10,4% e 37,5% dos casos foram classificados como moderados/severos em relação às variáveis ceratose, acantose e infiltrado inflamatório, respectivamente. Notou-se, também, 52,1%, 54,2%, 12,5%, 39,6% e 68,8% dos casos exibiu, respectivamente, faixa de infiltrado inflamatório, projeções papilares, dentes em serra, degeneração da camada basal e corpos de Civatte. Não houve associação significativa entre o tipo de lesão e as variáveis sexo, idade, local da lesão e hábito de fumar (p>0,05). Não houve associação significativa entre o tipo de lesão e as variáveis histológicas (p>0,05), bem como entre os tipos de lesão e as variáveis morfométricas (p>0,05). Também não houve diferença significativa entre variáveis clínico-demográficas e as variáveis morfométricas (p>0,05). Conclusão: A presente investigação não evidenciou associação entre lesões brancas e vermelhas de LPO e as suas características histomorfométricas. Entretanto, sugere-se que sejam realizados novos estudos com o uso da morfometria com a finalidade de se investigar se há diferenças histopatológicas entre tais lesões que possam definir seus diferentes comportamentos biológicos.
Introduction: Oral lichen planus (OLP) is considered to be a potentially malignant oral disorder with the possibility of varied clinical manifestations, histologically characterized by the presence of a strip of subepithelial inflammatory infiltrate and by degeneration of the basal layer of the epithelium. The presence of the keratin layer and Civatte bodies can also be identified. The diagnosis of OLP is clinical and pathological, which can generate subjectivity. It is hypothesized that the use of histomorphometry could contribute to a more objective analysis for the diagnosis of these lesions. Objective: To verify the association between the groups of white and red lesions and histomorphometric differences in OLP lesions. Methods: It was a retrospective cross-sectional study, in which 48 histological reports and sections were analyzed, which belonged to the collection of two distinct laboratories of Higher Education Institutions, and which were subjected to evaluations from the point of view of morphological variables and morphometric measures. Histomorphometric analyzes comprised the measurement between the basal distance to the epithelial surface, the distance from the apex of the epithelial crest to the epithelial surface, distance between the epithelial ridges, width of the epithelial crest, thickness of the keratin layer, thickness of the subepithelial band of the inflammatory infiltrate, extent and depth of the ulcer. Results: It was observed that 81.2% of the sample was female and 50.0% were under 50 years old. As for the injury site, it was observed that in 58.3% of cases it occurred in the jugal mucosa, with 83.3% of the sample showing a white lesion and 18.8% of the sample were from smokers. It was noted that 10.4%, 10.4% and 37.5% of the cases were classified as moderate/severe in relation to the variables keratosis, acanthosis and inflammatory infiltrate, respectively. It was also noted that 52.1%, 54.2%, 12.5%, 39.6% and 68.8% of the cases exhibited, respectively, a band of inflammatory infiltrate, papillary projections, saw teeth, degeneration of the basal layer and Civatte bodies. There was no significant association between the type of injury and the variables sex, age, injury site and smoking habit (p>0.05). There was no significant association between the type of lesion and the histological variables (p> 0.05), as well as between the types of lesion and the morphometric variables (p> 0.05). There was also no significant difference between clinical-demographic variables and morphometric variables (p> 0.05). Conclusion: This investigation did not show any association between white and red lesions of OLP and their histomorphometric characteristics. However, it is suggested that further studies be carried out with the use of morphometry in order to investigate whether there are histopathological differences between such lesions that may define their different biological behaviors.
