Título: Significado da liberdade e da responsabilidade pelo cuidar de si do cliente em hemodiálise

Abstract: Este estudo emergiu da reflexão acerca das minhas vivências, como estudante e profissional de enfermagem, atuante há cerca de 11 anos, junto aos clientes portadores de Insuficiência Renal Crônica (IRC), que são submetidos a tratamento de Hemodiálise (HD). As inquietações quanto à necessidade de promover o cuidado humanizado, baseado na inter-relação, compreensão e no respeito, com efetiva participação dos clientes, conduziram-me a definir como objeto de estudo: Significado da liberdade e da responsabilidade pelo cuidar de si para o cliente em hemodiálise. A escolha de o termo cuidar de si deu-se por considerá-lo não apenas ações de autocuidado, mas espaço de reflexão e de exercício da liberdade e da responsabilidade do cliente frente a sua existência. Utiliza uma abordagem existencialista pautada na Análise Existencial de Viktor Emil Frankl e em referencial teórico sobre o cuidar. Considerando tratar-se de uma pesquisa exploratória descritiva, estabeleci como objetivo: compreender o significado da liberdade e da responsabilidade pelo cuidar de si do cliente em hemodiálise. Assim, a Fenomenologia foi usada como método para análise. A coleta de dados foi realizada em um Hospital Público da cidade de Salvador, na Bahia, e foram depoentes clientes com diagnóstico de IRC que realizavam hemodiálise há mais de seis meses, e que, após serem devidamente esclarecidos, concordaram livremente em participar do estudo. A técnica de coleta dos depoimentos foi a entrevista fenomenológica. Na compreensão dos significados à luz da Análise Existencial de Viktor Frankl, emergiram as categorias: liberdade limitada pela responsabilidade diante da HD; liberdade e responsabilidade vivenciadas em situações que envolvem valores; responsabilidade frente à tridimensionalidade do ser cliente em HD cuidando de si; vazio existencial e sentido da vida. Ao penetrar na estrutura do fenômeno compreendi que os clientes vivenciam a liberdade como algo perdido, situado e limitado pela responsabilidade requerida pelo tratamento de HD. Frente a essa responsabilidade emergiram valores de atitude e de experiências, não tendo sido revelados valores criativos, mas sim, a falta de confiança na capacidade de modificar a situação em que vivem. Mantêm, através da fé em Deus, a esperança de sobreviver e de obter a cura, contudo, a falta de sentido da vida os conduz a vivenciarem a frustração, o vazio de autovalor e o vazio existencial. O sentido da vida foi desvelado através da vivência, quer do tratamento como meio restaurador da saúde, quer do amor dedicado à família. Durante o estudo, retornei freqüentemente às minhas inquietações do cotidiano profissional, ao ser enfermeira que cuida do cliente em tratamento hemodialítico. Neste sentido, fez-me repensar minha prática, permeando-a, ainda mais, com sentimentos de respeito à liberdade e à responsabilidade exercida pelos clientes frente às situações diárias impostas pela vida. A compreensão alcançada por este estudo tem possibilidade de contribuir como subsídio às discussões e reformulações de currículos e disciplinas do curso de enfermagem para que contemplem o cuidar, promovendo um planejamento de atividades onde o ser cuidado possa exercer sua liberdade como ser existencial.
This study stemmed from the reflection upon my experiences as a student and professional of nursing over eleven years with carriers of chronic renal failure that are undergone hemodialysis treatment. The restlessness regarding the need to foster the humanized care based upon interrelation, comprehension, respect, as well as an effective participation of the patients has led me to define as aim of research. The meaning of freedom and responsibility for the care of himself or herself in the hemodialysis patients. The choosing of the term care of ourselves has been given for considering it not only actions of self care, but space of reflection and exercise of freedom and the responsibility of the client before his or her own existence. This study utilizes an existentialist approach grounded on the Viktor Frankl existential analysis and on a theoretical reference on the care. Considering an exploratory descriptive research, I have set a goal: To understanding meaning of freedom and responsibility of the care of himself or herself of the hemodialysis patient. That way a phenomenology was used as a method of analysis. The data collection was performed in a public hospital in the city of Salvador – State of Bahia. Were interviewed patients with chronic renal insufficiency who underwent hemodialysis treatment for more than six months, and that after having been duly clarified agreed freely to attend the study. The technique utilized to collect the data was the phenomenological interview. In the comprehension of the meanings in the light of the existential analysis of Viktor Frankl, arose the categories: limited freedom for responsibility before the hemodialysis; freedom and responsibility experienced in situation that involve values; responsibility in the face of the three-dimensionality of being a hemodialysis patient taking care of himself or herself; existential emptiness and sense of life. In the structure of phenomenon I understood that hemodialysis patients experience the freedom as something lost, situated and limited before the required responsibility by the hemodialysis treatment. Before this responsibility emerged values of attitude and experiences not unfolding creative values, but the lack of confidence in their capacity of changing the situation in which they live. They keep through faith in god, the hope to survive and get the cure, however, the lack of sense of life lead them to experience the frustration, the emptiness of self value and the existential emptiness. The experience of the sense of life emerged through the treatment as a restorative means of health and the love dedicated to the family. Over this study I returned often to my inquietude regarding the everyday life of the professional of nursing that takes care of the hemodialysis patient. In this sense, it made me rethink my practice, filling it much more with feelings of respect to the freedom and the responsibility exerted by the patients before daily situations imposed by life. The understanding reached for this study has possibilities to contribute as a subsidy to the discussions and reformulations of résumés and subjects of the nursing course that contemplates the care, fostering a planning of activities where the being cared for cam exert their freedom as an existential being.
