Título: Farmacoeconomia da anfotericina b lipossomal. Da terapia antifúngica à leishmaniose

Abstract: RESUMO A anfotericina B (AmB) possui elevada potência leishmanicida e antifúngica. No entanto, está associada a uma elevada incidência de efeitos adversos sérios, que é significativamente reduzida com sua incorporação em lipossomas, com a obtenção da anfotericina B lipossomal (L-AmB). Essa tecnologia, por sua vez, possui custo elevado, o que limita seu uso, principalmente em cenários de recursos escassos. Nosso objetivo foi avaliar a custo-efetividade da anfotericina B lipossomal no tratamento de infecções fúngicas e da leishmaniose tegumentar americana (LTA). Na terapia antifúngica, L-AmB foi comparada à caspofungina (CASP) em uma revisão sistemática de avaliações econômicas que incluiu análises de custo-efetividade (ACE) no tratamento de micoses sistêmicas de pacientes gravemente doentes. Nessa revisão a análise da qualidade possibilitou a inclusão de oito estudos de boa qualidade e em seis deles, CASP foi a alternativa dominante. Dessa forma, CASP foi considerada custo-efetiva no tratamento de pacientes com infecções fúngicas severas, por produzir taxas de efetividade semelhantes e custo total de tratamento significativamente inferior, em comparação à L-AmB. Para o tratamento da LTA, foi realizada uma análise de custo-efetividade na qual L-AmB foi comparada à AmB convencional e ao antimonial pentavalente. Um modelo markoviano, construído com dados retrospectivos de pacientes internados em um hospital do nordeste brasileiro, demonstrou que a L-AmB pode ser uma alternativa custo-efetiva para o tratamento da LTA. As análises de sensibilidade demonstraram que o valor unitário da L-AmB foi a variável de maior impacto nas medidas da Razão de Custo-Efetividade Incremental (RCEI) e que o tamanho do benefício obtido com a L-AmB depende de uma revisão de dois parâmetros principais: o valor de aquisição do medicamento e a disposição-a-pagar pelo gestor em saúde, por paciente tratado. ABSTRACT. Amphotericin B (AmB) is a highly potent leishmanicide and antifungal drug. However, it use is associated with a high incidence of serious adverse effects, which are significantly reduced by the incorporation of AmB into liposomes, that results in the liposomal amphotericin B (L-AmB). That technology, in turn, has a high cost which limits the use of L-AmB, especially in settings of scarce resources. We aimed to evaluate the cost-effectiveness of liposomal amphotericin B in the treatment of fungal infections and mucocutaneous leishmaniasis (MCL). In antifungal therapy L-AmB was compared to caspofungin (CASP) through a systematic review of economic evaluations which included cost-effectiveness analysis (CEA) in the treatment of systemic fungal infections in severally ill patients. During the systematic review, the analysis of quality becomes possible to include eight studies of good quality. In six of the included studies, CASP was the dominant alternative. Thus, CASP was considered cost-effective in the treatment of patients with severe fungal infections, because produced similar effectiveness rates and significantly lower the total cost of treatment when compared to L-AmB. For the treatment of MCL, a cost-effectiveness analysis in which L-AmB was compared to conventional AmB and pentavalent antimony was conducted. A Markov model was built with retrospective data from inpatients of a hospital in Brazilian northeastern. The model demonstrated that L-AmB could be a cost-effective alternative for the treatment of MCL. The sensitivity analyzes showed that the unit value of L-AmB was the highest impact variable on measures of Incremental Cost-Effectiveness Ratio and the benefit size derived from the L-AmB depends on review of two main parameters: the purchase price of the drug and the willingness-to-pay by health manager for each treated patient.
